With a vision to provide accessible healthcare for all in Fond du Lac and Dodge counties, Agnesian HealthCare offers Samaritan Health Clinic services at both St. Agnes Hospital and Waupun Memorial Hospital.
The clinic's mission is to provide quality healthcare to the uninsured/low income patients within these counties and to be a force for a healthier community.
The Samaritan Health Clinic provides the following services:
- Non-emergency medical care
- Emergency dental care (Fond du Lac only)
- Laboratory tests
- Pharmacy service
- Blood pressure screenings
- Health information and education
- Community resource information
Samaritan Health Clinic services are available to those who qualify. Individuals can be working a part time or full time job at minimum wage and are not offered insurance through their employer. It can be young adults who no longer qualify to be covered under their parents insurance. They can be individuals living at a homeless shelter or recovery centers It could be a next door neighbor or a family member who may have fallen on hard times.
We offer primary non-emergency care through our Agnesian Care Clinic. Vision care is provided through the Fond du Lac Regional Clinic Eye Care department and Optical Shop. Patients also have access to services provided by Consultants Laboratory, Family Foot Clinic and radiology services with referral from our primary care providers.
Patients must be a resident of Fond du Lac County (St. Agnes Hospital), or Fond du Lac and Dodge counties (Waupun Memorial Hospital). They must have no insurance, including Medicaid, Medicare or BadgerCare, and must have income at or below 165 percent of the federal poverty level. They must also be 19 years of age up to 64 years of age.
