Hospice Hope provides comfort and enhances the quality of life for patients and their families. Whether in your home, a contracted CBRF or nursing home or Hospice Home of Hope, families who want hospice care will receive routine services regardless of insurance or financial resources. Hospice Hope provides comprehensive services directed toward keeping the patient at home:

  • Physician directed medical care
  • Skilled nursing care
  • 24-hour on-call service
  • Spiritual counseling
  • Medical social services
  • Bereavement support to family
  • Volunteer support system
  • Equipment and medical supplies
  • Medications for pain and other symptom management
  • Continuous nursing care at home for short periods of crisis

Hospice Home of Hope

Agnesian HealthCare's Hospice Home of Hope is a residential facility providing palliative care to individuals diagnosed with a terminal illness with a life expectancy of less than six months. The 9700-square-foot facility includes 12 private, home-like patient rooms; sunroom and porch areas; a common family/TV area; meditation room and a large living room with a fireplace. A staff of registered nurses, attending physicians, nursing assistants and social workers provide comfort to patients when a cure is not possible.

90% of people who seek care at Hospice Home of Hope will experience a wait. While this is never long, it is not an option. To meet these needs, our eight room expansion is underway. The goal is to be open late summer 2017. We need your support to make this goal a reality.
