A friendly smile….the touch of a hand….a word of encouragement. These are just a few of the reasons the guests at St. Francis call this place their home. Our tradition of caring started in 1903, and since then we have grown into a comprehensive facility offering three levels of care:
- Skilled nursing care: St. Francis Home is the heart of its care campus. This 106-bed Medicare-certified skilled nursing facility specializes in long-term care for the elderly. It provides the latest in nursing home care, stressing a homelike atmosphere for residents and offering a special unit dedicated to the care of residents with Alzheimer's disease.
- Independent living: St. Clare Terrace offers independent living options. This safe, secure community living environment provides residents a fulfilling lifestyle in their retirement years.
- Assisted living: St. Francis Terrace offers assisted living care. This is an interim care option that provides some nursing support and assistance, yet allows people to live independently. St. Francis Home has expanded its assisted living capacity with 42 new apartments.

The mission of St. Francis Home is to provide a caring Christian home environment for older adults. Under the sponsorship of the Congregation of Sisters of St. Agnes, we strive to serve each resident with respect, dignity and a sense of individual worth.

In addition to St. Francis Home, Agnesian HealthCare’s Adult Day Services may be an option for your loved one. Adult Day Services is a day program that helps meet the healthcare, personal care and socialization needs of the elderly, chronically ill and disabled in the Fond du Lac area. It is located at 420 E. Merrill Avenue, Fond du Lac, and is open Monday through Friday from 7:30 a.m. to 5:30 p.m. Call Adult Day Services at 921-5812 for more information or a tour.
