Home-Delivered ServicesAgnesian HealthCare knows how important it is for patients to receive certain healthcare services in the comforts of their own home. That is why Home-Delivered Services offers a number of services to help anyone from newborns to older adults who need assistance to remain at home. Hospice Hope, Home Care, Home Infusion Therapy, Home Oxygen, Home Medical Equipment and Lifeline are available in Fond du Lac, Dodge and Green Lake counties. The Hospice Home of Hope residential facility is located in Fond du Lac. These services are available throughout Agnesian HealthCare's service area.
Located at 239 Trowbridge Drive in Fond du Lac, Hospice Hope covers a 30-mile radius of Fond du Lac. Green Lake Hospcie is located at 745 South Street, Suite #4, Green Lake, and covers a 40-mile radius of Green Lake. Hospice Home of Hope Agnesian HealthCare's Hospice Home of Hope is a residential facility providing palliative care to individuals diagnosed with a terminal illness with a life expectancy of less than six months. The 9700-square-foot facility includes 12 private, homelike patient rooms; sunroom and porch areas; a common family/TV area; meditation room and a large living room with a fireplace. Home Infusion Therapy Home infusion is most any type of IV (intravenous) therapy, such as antibiotics, long-term chemotherapy, steroid therapies, pain management and IV nutrition, which patients receive at home or work. By receiving therapy at home, patients are more comfortable and can reduce the cost of care.
