Statistics show that the more a person is involved in his or her own care, the fewer complications will occur. Our nationally recognized and certified diabetes self-management education program (through the American Diabetes Association) can help you with the daily management of diabetes.
Our certified diabetes educators (certified with the National Certification Board of Diabetes Educators) work with your healthcare provider to develop your individualized diabetes management plan at sites located in Fond du Lac, Ripon and Waupun. Our staff includes both registered nurses and registered dietitians to meet your specific individual needs.

Continuous Blood Glucose Monitoring
This data collection tool can show what a glucose finger stick cannot: 72 hours of continuous data. You are able to enter meals, exercise and medication to see the effect these events have on glucose control. Medicare has recognized a reimbursement code for this.
Cost of Services
Diabetes Services charges are covered by most health insurance policies and Medicare. Contact your insurance provider to verify coverage. In addition, our associates can help individuals identify financial resources that are available in the management of diabetes.
All registration and billing services are provided through Waupun Memorial Hospital for services at Waupun and St. Agnes Hospital in Fond du Lac, and through Ripon Medical Center for services at Ripon.
