What is an anticoagulant?

An anticoagulant is a medication that affects the blood clotting process.
Common oral anticoagulants include warfarin (Coumadin®), aspirin, dabigatran (Pradaxa®) and clopidogrel (Plavix®). Common injectable anticoagulants include dalteparin (Fragmin®), enoxaparin (Lovenox®), and heparin.

Why was Anticoagulation Management Service established?

If you are currently taking warfarin, the Anticoagulation Management Service is an important health resource for you. Studies have shown that patients who participate in anticoagulation management clinics have a much lower incidence of heart attacks, strokes, and bleeding complications when compared to patients who are cared for in the usual manner.

Who is Eligible to Use the Anticoagulation Clinic?

Anyone who requires anticoagulation therapy with a referral from their health care provider. Once your provider has referred you to the anticoagulation management clinic, associates at the clinic will contact you to set up your appointment.

What Happens When I'm First Enrolled in the Program?

All patients meet with a specially-trained clinician and are educated on why they are on anticoagulation therapy, how the anticoagulation management clinic works, the actions of their anticoagulation medication, its side effects, and drug and dietary interactions. A dosing schedule is discussed and plans are made for when you will return for follow-up visits.

How Is My Therapy Monitored?

Warfarin (Coumadin) therapy is monitored by using the latest technology. Anticoagulation clinicians will obtain a small blood sample from your finger tip to measure the International Normalized Ratio (INR). The test results are immediate, thus our clinicians are able to review the results, discuss any outside factors affecting the results and make dosage adjustments before you leave the clinic. All this is completed in minutes during a scheduled appointment time.
