As a nurse practitioner, she provides diagnostic, therapeutic and preventive healthcare services; takes medical histories; examines patients; orders and interprets laboratory tests and X-rays, and makes diagnoses; treats minor injuries by suturing, splinting and casting; performs pre-employment physicals; oversees worker’s compensation and disability cases; and serves as case manager, coordinating medical services to ensure quality and cost-effective care at each stage of treatment. “I strive to serve our community and help people live healthier and happier lives,” according to Chouinard. “I would like to become involved in the community by working with the area companies to help bring wellness programs to their employees. I enjoy teaching and educating about wellness.” Chouinard earned her bachelor’s degree in nursing from the University of Wisconsin – Oshkosh, and her master’s degree as a family nurse practitioner from the Chamberlain College of Nursing Family Nurse in Addison, Illinois. Chouinard and her husband live in Beaver Dam with their two children. In her free time, she enjoys fishing, camping, watching the Packers and going to the beach.
