Center for Physical Rehab

Since 1979, the 12-bed Center for Physical Rehab, located at St. Agnes Hospital, has been serving the greater Fond du Lac area.

St. Agnes Hospital's Center for Physical Rehab provides a comprehensive rehabilitation program for patients with disabling conditions such as head injuries, stroke, hip fractures, joint replacements (hip, knee), amputations, traumatic and non-traumatic brain injury, spinal cord injuries, multiple trauma/medically complex injuries, Parkinson's disease, Guillain-Barre syndrome, multiple sclerosis, and other neurological and neuromuscular conditions.

Services provided by the Center for Physical Rehab include:
* Medical direction from neurologists who specialize in rehab
* 24-hour rehab nursing care and education
* Physical, occupational, speech, recreation and aquatic therapies
* Nutrition services
* Social Services
* Rehab counseling
* Pastoral Care services
* Referrals to orthotics and prosthetics
* Motovate – a biofeedback system
* Stroke screenings for the community
* Referrals to post-rehab services (such as vocational rehab, Adult Day Services, Transitional Care Unit, outpatient therapy, home care, Meals on Wheels, durable medical equipment and aquatic exercise).
* Froedtert Hospital partnership

The center is accredited by the Commission on Accreditation for Rehabilitation Facilities (CARF), which recognizes organizations that show substantial fulfillment of the standards established by CARF and clearly indicate that present conditions represent an established pattern of total operations that is likely to be maintained and/or improved in the foreseeable future. It is also accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
