Transitional Care Unit

St. Agnes Hospital's Transitional Care Unit is a multidisciplinary collaboration of all providers and services in an effort to streamline patient care as it transitions from the hospital to home. This service can be beneficial for individuals with chronic illnesses, or patients hospitalized for common medical and surgical conditions. Transitional Care takes steps to assist recovery and reduce the need for acute hospital and emergency department use, and prevent health status decline. Transitional Care Unit services include the expertise of physicians, nurses, social workers, discharge planners, pharmacists and other members of the healthcare team.
