Benefits

Medical/Prescription, Dental and Vision Insurance*

Coverage will begin under the Agnesian HealthCare plan the first day of the month following one full month of employment. You will have the opportunity to elect a plan and level that best suits your needs. All Primary Care Medical Plan services must be provided by an Agnesian HealthCare provider to be eligible for benefits. (Includes, but not limited to: Family Practice, OB/GYN, Pediatrics, Internal Medicine, Physical Therapy, Occupational Therapy and Speech Therapy)

Life/AD&D Insurance*

Agnesian HealthCare will provide a base level of life insurance and accidental death & dismemberment coverage to you on the first day of the month following 90 days of continuous employment at no cost to you. You may select to purchase additional life insurance coverage for you, your spouse and/or children that provides additional protection to the company paid portion.

Disability Insurance*

Agnesian HealthCare will provide short term and long term disability insurance to you on the first day of the month following one year of employment at no cost to you. The short term disability plan will cover you for 50% of your base salary to a maximum of $1500 per week after 60 days of disability. The long term disability plan will cover you for 50% of your base salary to a maximum of $6000 per month after 180 days of disability.

Paid Time Off**

Eligible employees who have successfully completed the 90 day orientation period are eligible to use Paid Time Off (PTO) benefits that have accrued from the first date of employment. PTO benefits are to be used for the traditional vacation, holiday and individual sick days and are accrued based on years of service. Eligible employees can elect to maintain their normal paid time off schedule or elect to buy or sell PTO hours.***

Reserve Sick Leave**

Eligible employees who have successfully completed the 90 day orientation period are eligible to use Reserve Sick Leave (RSL) benefits due to an extended illness or injury (more than 3 days). RSL hours are accrued from the first date of employment.

Flexible Spending****

Health Care Reimbursement Accounts (HCRA) and Dependent Care Reimbursement Accounts (DCRA) are available to employees on the first day of the month following one full month of employment. Flexible spending accounts allow you to pay for eligible health care expenses and/or for dependent day care expenses incurred so you can work with tax-free dollars.

Tuition Assistance* / Education Days****

After 90 days of employment, eligible employees can use $2500 per calendar year for tuition assistance. The course should be directly related to a position within the Agnesian HealthCare ministry. Successful completion requires a grade of "C" or above in a graded course. Employees who complete their approved course will be required to work for Agnesian HealthCare for one year or repayment will be required. In addition, a specified number of education days may be used for on or off-site seminars related to your position.

401(k) Plan

All employees are eligible to participate in the Agnesian HealthCare Retirement Savings Plan Plus if you are at least 18 years old. You may defer as much as you would like up to the IRS limit. Upon hire, there is a 30-day period in which you can call the plan administrator to enroll or opt out of the plan. If you do nothing, you will be automatically enrolled in the plan at a 2% deferral rate. Agnesian HealthCare matches 50% of the first 5% of your contribution to the plan. You are immediately vested in your elective deferral and the company matching contributions. On top of that, each year, Agnesian HealthCare may contribute an additional percentage of your total compensation provided that you have completed 1000 hours of service and are employed on the last day of the plan year.

Employee Assistance Program (EAP)

All employees and family members within the household have access to free, confidential individual or family counseling through Agnesian HealthCare’s EAP program.

Employee Discounts

All employees are eligible to receive various discounts to: any Agnesian Pharmacy/Health Shoppe, Optical Shop, Audiology Department, St. Agnes Gift Shop, Fond du Lac YMCA or Mayville TAG Center. Present your photo ID badge to receive the discount(s).

Employee Credit Union

All employees have the option to open financial accounts through the employee credit union (Fond du Lac campus).

Children's Choice Day Care

All employees are eligible to utilize child care services through Children’s Choice Learning Center (Fond du Lac campus).

Employee Fitness Center

All employees are eligible to join The Zone, Agnesian HealthCare’s fitness center at a low monthly cost, based on the length of your contract (Fond du Lac campus). Employees may also use the Waupun Memorial Hospital Rehab Gym, free of charge, dependent upon patient scheduling.

Employees Recruiting New Employees (ERNE)***

Any employee who refers an external applicant for any position within Agnesian HealthCare becomes eligible to receive a referral bonus. The bonus amount will be established by the full time equivalency (FTE) in which the candidate is hired. Each entity reserves the right to determine which positions are applicable for this program.

Wellness Initiatives

All employees have access to free Health Risk Appraisals, flu shots and participation in many wellness programs throughout the year.
