February 18, 2021 Update

SSM Health is celebrating the imminent opening of its new 40,000-square-foot physician offices in Beaver Dam with a virtual ribbon-cutting ceremony and tour of the facilities. The new offices, located just off Highway 151 at 130 Corporate Drive, will open to patients on Monday, February 22.

 “We’ve enjoyed a long relationship with the people in and around this thriving community, which is why we’ve invested so much in building this beautiful new facility,” says Damond Boatwright, SSM Health regional president. “We’ve invested in new services, new technology and conveniences to improve access for primary care and the many kinds of specialty care that might be needed.”

The new facilities will house 30 primary and specialty care providers, an on-site prescription center, laboratory and medical imaging services. The new building also offers a private space available for telehealth visits for people who may not have easy access to internet or transportation services.

“We’re confident that our existing patients, as well as those looking for local care, will be impressed with the attention to detail we’ve paid with our offices,” Boatwright adds. “We took great care to create a calm and welcoming feeling throughout the building, using natural light and open spaces to enhance the healing environment.” The new facility also showcases the talents of local artists who have captured many of the scenic landmarks within the historic community.

The Beaver Dam Area Chamber of Commerce plans to host the virtual ribbon-cutting ceremony on its Facebook page on Monday, February 22.

January 22, 2021 Update

With roughly one more month before the first patient walks in the door at the new SSM Health physicians offices in Beaver Dam, work is focusing less on actual construction and more on the installation of furniture and fixtures needed by providers and team members to provide compassionate and quality care.

At mid-January, efforts surrounded the care team areas on both first and second floors as furniture installation begins to clearly define various workstations and workspaces. Installs first focused on erecting furniture walls/partitions and will then move into desks and partial walls for specifics work areas.

Exam rooms are beginning to take shape as mobile work surfaces and furniture is being added to house computers, phones and other items needed by providers.

Major construction on the clinic’s second floor is complete and construction team members are focused on punch list items to be completed prior to the move.

Flooring and vinyl base installation is being completed on the first floor, along with painting. However, construction efforts are winding down nicely.

Information technology (IT) devices are now on-site and placement is underway. Device installation will begin in the exam rooms and then follow the furniture installation crews as they complete workstation installs. Elevator components have arrived and installation is nearing completion.

Waiting room and breakroom furniture, depending on location, will arrive in separate waves, followed by artwork and wayfinding installation.

In late January, the first transfer of equipment will begin, including supplies that can be physically placed by staff members. Mobile supply carts will be positioned for each exam room. Procedure room lights are in place.

A new Siemens DR X-ray machine has been installed. The clinic will be transitioning its EKG machine to the new location and is planning to secure a second.

SSM Health will be sharing more details about the new clinic through marketing efforts, including radio, print, outdoor and direct mail.

December 18, 2020 Update

The community will get its first public look at the new SSM Health physician offices in Beaver Dam during a virtual tour starting at 9 a.m. on Sunday, December 20. For those interested in viewing, they can visit either the SSM Health Dean Medical Group or Agnesian HealthCare Facebook pages or ssmhealth.com/beaverdam, or agnesian.com/beaverdam.

“We’re excited to give this initial sneak peek to our patients and community at large in a safe manner,” according to Matt Bagley, clinic manager of the SSM Health Dean Medical Group - Beaver Dam & Columbus. “We’re very proud of how far we have come in a quick timeframe, and we know that those individuals and families that we support will be impressed by what they see.”

Work continues right on schedule. By the end of December, the second floor will be complete, including all painting, flooring, cabinetry and door installation. It will be ready for furniture, information technology and medical equipment installation in January and February.

Elevators are scheduled to arrive in mid-December and will be functional by the end of January 2021.

On the building’s exterior, the construction crew is close to finishing wood appearance paneling to complement existing stone and brick. In December, the SSM Health logo will be installed on three different sides of facility, including the main entrance, facing Highway 151 and south-facing as travelers drive from Beaver Dam north to Waupun and beyond.

Now that the work is concluded on the second floor, the first floor will be a core focus. “December will be spent on final painting, flooring, cabinetry installation and lighting fixtures, as well as ceiling work,” according to Kyle Prochaska, senior construction project manager for SSM Health Dean Medical Group. “By the end of January, construction will be complete and furniture installation will start.”

The facility features natural lighting with breathtaking views and will feature motorized blinds and glazing for privacy. Artwork by a local artist will showcase the Beaver Dam area’s natural beauty to complement the facility’s healing environment.

December 8, 2020 Update

Construction efforts remain on schedule at the new SSM Health clinic in Beaver Dam. With the weather turning colder, most of the work is focused on the facility’s interior, however, some exterior work continues.

The construction crew is installing wood appearance paneling to complement existing stone and brick. Work crews are finishing up sidewalks and will install trees and a few other plantings at the front of the building with additional landscaping and parking lot work to follow in the spring of 2021.

Inside, the clinic is really beginning to take shape. “By the end of November, all drywall has been installed in the facility,” according to Kyle Prochaska, senior construction project manager for SSM Health Dean Medical Group. “For interior work, we started from the top down. The second floor is falling into place nicely and now we are finishing drywall install on the first floor and starting to paint.”

Cabinets and drawers are being delivered for construction teams to begin install. Work teams also are placing ceiling grids so they can install ceiling tiles once all the mechanical and electrical work is completed. “As it relates to our interior work, November into January will be a most impactful time,” Prochaska says. “December will be spent on flooring and lighting fixtures, as well as ceiling work.”

November 4, 2020 Update

It’s easy to see that the exterior of the new SSM Health clinic under construction off Highway 151 in Beaver Dam is taking shape. What is less apparent is the extensive work going on inside.

“At the beginning of October, you could stand at one end of the facility and see all the way to the opposite end,” says Kyle Prochaska, senior construction project manager for SSM Health Dean Medical Group. “Right now, that is no longer the case. In fact, as of mid-October, 90 percent of the second-floor drywall has been installed with the first level following behind.”

By end of October, Prochaska says crews can begin interior painting since the drywall process has been right on task.

Outside curb, gutter and sidewalk work is nearing completion with a plan to apply a base coat of asphalt this month.

The exterior brick work for the clinic’s second floor is complete and workers are placing the stone for the first floor and installing framing in preparation for the facility’s beautiful glass windows. The majority of the exterior glazing will be completed in October.

“While we have an aggressive construction schedule,” Prochaska says, “we are progressing right where we want to be which is a real testament to the design and construction teams. We’re entering the stage in this project where fun addition will come to light such as interior finishes like painting, flooring and specialty touches.”

September 29, 2020 Update

A recent onsite review of construction with the clinical team that will occupy the new SSM Health clinic off Highway 151 in Beaver Dam is validating that all plans put in place prior to the project’s physical start are right on track.

A group consisting of providers, care team members, operations and facilities met onsite on September 21 to tour construction progress. The team was able to visualize what the completed facility will look like and envision workflows in the new environment.

“It was gratifying to have our clinical managers, leaders and care team members, as well as other project collaborators, actually onsite to review what was originally noted in our two-dimensional plans and to validate that all the planning efforts will meet our specific needs at time of opening,” according to Kyle Prochaska, construction project manager - senior, SSM Health Dean Medical Group.

The onsite audit indicated that all plans and steps are still within project scope and only a few minor adjustments need to be made. “This onsite event was important because once drywall is up, it’s more costly and time consuming to make changes. The review verified our planning is in line with the actual construction and end use.”

The new 40,000-square-foot, standalone clinic has an anticipated first day of patient care in February 2021.

Roofing work is nearing completion signaling another important milestone. “This is significant step as it ensures we are weather tight mitigating risk of schedule delays due to inclement weather.”

By the end of September, masonry and stone work will be nearing completion, having started from the building’s second floor to the first floor. During the same time, frames will be installed in preparation for the facility’s beautiful glass windows.

“For those driving by the facility, they will begin to get a true sense and feeling for what this new facility will offer, including its healing and inviting environment,” Prochaska says.

In October, curb and gutter work will be underway, with asphalting of the approximate 200 parking stalls. Some landscaping will be complete, including some planting and seed work. Trees and other larger landscape pieces will be planted next spring.

Much work is progressing in the building’s interior. Mechanical, electrical and plumbing work is underway, which will then lead to drywall installation and finishes.

September 8, 2020 Update

If you drive by the new SSM Health clinic off Highway 151 in Beaver Dam within the next few weeks, you will see the beauty of the facility’s brick stone and glass exterior. Construction crews have also begun the roofing work to enclose the building keeping it dry for interior work to begin in earnest and remain on schedule.

The new 40,000-square-foot, standalone clinic has an anticipated first day of patient care in February 2021. This important project represents the meaningful partnership between SSM Health Dean Medical Group and our Greater Fond du Lac ministry.

“Once the roof is complete and the glass is in place, weather will not factor into our schedule as it does today,” according to Kyle Prochaska, construction project manager - senior, SSM Health Dean Medical Group. “With these developments, our community will get a better sense of what this new facility will offer to the Beaver Dam area.”

Masonry and stone work have begun on the facility’s north side and will proceed around the facility exterior. This work will continue through September and into October. This is an exciting development as its healing and inviting environment will begin to unfold to those who drive by.

The interior of the building is also taking shape. Framing is complete, and the mechanical, electrical and plumbing work is underway. This work will be a focus during September and October before moving into drywall installation and finishes.

The new clinic will house SSM Health primary and specialty care providers, as well as imaging, laboratory and pharmacy services. Once built, SSM Health will move our services currently provided in leased space adjacent to Beaver Dam Community Hospital to the new location.

"We are excited about the opportunities this new clinic has to offer. Its location in the 151 Business Park will make for easy access off the highway for patients who may be traveling from outside of the Beaver Dam community, and the upgraded facilities and equipment will make for a better patient experience," Damond Boatwright, regional president for SSM Health - Wisconsin.

The project is ahead of schedule and on track to move to interior work by the fall. An anticipated opening is set for early spring 2021. So far, the steel erection of the building is complete and concrete deck and slabs are being completed. At this time, the building will become enclosed with the north, south and west sides being sheathed and insulated. Brick installation will begin. This work makes way for crews to finish the brick veneer and begin glazing the exterior finishes in August. By this fall, with the building's exterior largely finished, crews will move indoors to complete the interior.

With work moving quickly, the new location that will house both primary and specialty care has an anticipated first day of patient care in March 2021. More details on planned celebrations, open houses and other project milestones will be shared later in the year.
