How to Find Us  From the South
From Highway 41, take the Highway 151 North exit and turn right off the exit. From Highway 151 North, turn left onto Camelot Drive. The first driveway to your right is the Fond du Lac Surgery Center.
From the North
From Highway 41, take the Highway 151 exit. Turn left at the stop light. From Highway 151 North, turn left onto Camelot Drive. The first driveway to your right is the Fond du Lac Surgery Center.
From within the City limits
Take Main Street South past Pioneer Road and turn left onto Camelot Drive (just before the Citgo station). Follow Camelot Drive to the second driveway marked Fond du Lac Surgery Center.
From St. Agnes Hospital
Take Division Street East. Take a right on National Avenue. At the intersection of National and Fond du Lac Avenue near the Knights of Columbus Hall, turn left on County V. Cross over Pioneer Road/County VV. Turn right on Highway 151. Turn right on Camelot Drive. The first driveway to your right is the Fond du Lac Surgery Center.
