Agnesian HealthCare’s Fond du Lac Regional Clinic is pleased to welcome Kimberly Hornes, APNP, as a new cardiology nurse practitioner, located at 420 E. Division Street.
As a nurse practitioner, she provides care to patients with various heart concerns, such as coronary artery disease, atrial fibrillation and hypertension.
“I understand what it means to individuals to have an effective one-on-one partnership with their health care provider,” says Hornes. “As such, I treat every patient as if they were a family member. That’s so important as patients navigate various health concerns ensuring that their needs are being met.”
Hornes earned her bachelor’s and master’s degree in nursing from Marian University. She has a long history with Agnesian HealthCare starting in patient registration and admitting. Hornes also served as a certified nursing assistant and licensed practical nurse in both the St. Agnes Hospital dialysis department and GI/Endoscopy Lab, and then as a registered nurse on the hospital’s sixth floor and Cardiopulmonary Services.
“This is such a natural progression for me getting back into a provider role,” according to Hornes. “I have a passion for helping patients and I have seen so many aspects of the quality care that we offer through Agnesian HealthCare.”
Hornes is married with two children. She enjoys spending time with her family, gardening and traveling.
